Instructions for filling out the electronic pdf

1. Click on tab to open form. Form will appear in the window.

2. Go to your File menu and select print.

3. Save as a PDF to your hard drive.

 

Once on your hard drive:

 1. Open file by double clicking on it.

 2. Fill out the form and save it.

 3. Email back to: info@youluckydoghouston.com.

 

Don't forget to attach the file!
